This Policy sets out the basis, on which any personal data we collect from you, or that you provide to us, will be processed by us. Your consent We collect personal information from you in a variety of ways including, but not limited to, when registering a pet with the surgery, and using the services provided by Burnt Ash Veterinary Clinic. You will also be required to check your details on our GDPR consent forms that will be produced when you next visit the surgery and will be re validated on a yearly basis. You understand and agree that personal information you provide will be held on a database operated by us, and other selected third party being Teleos Management Systems. We will only collect personal information, when you submit such information to us. You can always refuse to supply personal information, but this may prevent you receiving veterinary services from us for your pet.
What information do we collect and why? Your title, first name or initial, surname, postal address, email address and contact telephone numbers. We will also require you to confirm the 1st line of your address over the telephone or in person so we can confirm that we have the correct client record card when you contact us.
Register you as a new client or update your details if they change., Make Appointments, Contact you about your pet, or your account.
Maintain a safe and secure service by helping to prevent and detect fraud. We may need to disclose information to assist in the treatment of your pet and to assist in legal or debt recovery processes.To help us respond to any issues or queries that you may have.
How we protect your information: We endeavour to protect your information by: Keeping your information, up to date and accurate. Please note that to do this we require you to tell us if any of the details are incorrect at the point of signing the GDPR validation form.
We have in place strict security procedures for the storage and disclosure of your information, to prevent unauthorised access.
Protecting your security: We do not hold any Credit or Debit card information on our database, Any card transactions are carried out using a non integrated PDQ machine which does not store any card details and the receipt printed and kept by us “merchant copy” only shows the last 5 numbers of the card that was used. We will always gather any personal information from you directly or from someone authorised by you (e.g. other veterinary clinics, insurance companies etc)
All information provided by you will be treated securely and strictly in accordance with the ACT all The data processors that we use at Burnt Ash Veterinary Clinic Limited being Microchip Companies, Clinical Laboratories, IT management System etc are GDPR compliant.
Statement on the use and disclosure of personal data.: We do not sell, trade, or rent personal information to others. We may need to use a third party service to help us to operate our business and administer activities on our behalf.
We may need to share your information with our out of hour’s emergency service if you were to use them, or if your pet is sent to a referral centre for more specialised treatment. If you were to change to another veterinary clinic we would need your consent to send over your pets’ history as your pets record is attached to your personal details.
Your rightsYou have the right to ask us not to process your personal data for marketing purposes. Burnt Ash Veterinary Clinic does not send email or text marketing, but make sure you tick the correct boxes on the GDPR consent form.
We will hold your personal information, for the treatment of your pet for 20 years, this is to take in to account geriatric pets and insurance claims. You have the right to ask us to delete your information that we hold on our system.
Access rights You have the right to access the personal data about you that is held by us, by contacting the surgery as a written request , there will be no charge for this, you will need to come in person to the surgery and you will need to bring photo identification before the information is given to you.
